Scheduling an appointment with our online appointment service is easy! All you have to do is fill out the form below and I will call you to confirm your appointment. Appointments are available Tuesday through Saturday, 9am to 4:45 pm and may be set up to two weeks in advance.
Cancellation policy: Please give 24 hour notice when you cancel an appointment. We reserve the right to charge a $20 fee for appointments cancelled less than 24 hours before the appointment time.
